Thank you for completing the secure contact form for DBT services.

Once we review the referral request, we will try our best to connect you with one of our available clinicians based on fit between your reasons for seeking services and a clinician’s scope of practice, as well as your preferences and availability for appointments. Certain services/clinicians may have a wait time.

Please be aware that while we aim to connect you with one of our clinicians/services, we cannot guarantee service provision.

The purpose of the referral form is to collect information only and does not establish a therapeutic relationship. By completing and submitting this form, you acknowledge that:

  • The information provided will be used solely for the intended purpose of referral.
  • No therapeutic relationship or services are implied by the completion of this form.
  • Confidentiality of the information provided will be maintained to the extent required by law and professional standards.

If you do not hear from us in the next 6 months, please do not hesitate to follow up with us.

IF YOU ARE IN DISTRESS, CALL OR TEXT 988.
